ELIGIBILITY CRITERIA for
LTC at Home Patients
If you are interested in availing the LTC-at-Home services, please note that you must meet the eligibility criteria. The criteria include:
Being enrolled in home and community-based services (HCBS) or other qualifying waiver programs
Taking multiple medications
Having three or more chronic conditions
Having limited mobility that makes leaving the home difficult
If you meet the criteria, you may be eligible for our LTC-at-Home services.
